Type 1 vs Type 2 Diabetes

Type 1 diabetes vs type 2 diabetes: differences in symptoms, tests and treatment

Short answer

In type 1 diabetes the immune system destroys the cells that make insulin, so the body makes little or none and insulin injections are needed from the start. In type 2 diabetes, which is far more common in India, the body does not use insulin well and gradually makes less of it; it is linked to weight, inactivity and family history. Type 1 often starts suddenly in children and young adults, while type 2 usually develops slowly in adults.

FeatureType 1 diabetesType 2 diabetes
CauseAutoimmune destruction of insulin-producing cells in the pancreasInsulin resistance plus gradually falling insulin production; linked to excess weight (especially belly fat), inactivity, family history
Typical symptomsExtreme thirst, frequent urination, rapid weight loss, hunger, tiredness; sometimes first noticed as DKAOften no symptoms for years; thirst, frequent urination, tiredness, slow-healing wounds, infections, blurred vision
Onset & timingUsually children, teenagers or young adults; develops over weeksUsually adults, increasingly younger in India; develops over years
Key testsBlood sugar, HbA1c, ketones; antibodies and C-peptide help confirm typeFasting sugar, HbA1c, glucose tolerance test; kidney, eye and cholesterol checks
Treatment approachInsulin for life, sugar monitoring, carbohydrate awareness, educationDiet, exercise, weight loss; tablets and sometimes injections, including insulin, as needed
Danger signsDiabetic ketoacidosis (DKA): vomiting, belly pain, deep fast breathing, drowsinessVery high sugar with dehydration and confusion; long-term damage to eyes, kidneys, nerves, heart

How doctors tell them apart

  • Age and speed of onset give the first clue, but either type can occur at any age.
  • Antibody tests support type 1.
  • C-peptide shows how much insulin the body still makes: low in type 1.
  • Ketones at diagnosis and rapid weight loss point towards type 1.
  • Some adults have a slow-onset autoimmune form (sometimes called LADA) that can be mistaken for type 2.

See a doctor urgently if

  • Vomiting, belly pain, deep or fast breathing, fruity-smelling breath (possible DKA)
  • Confusion, extreme drowsiness or fainting
  • Low sugar signs not improving: sweating, shaking, confusion after taking sugar
  • Sugar readings very high that will not come down
  • A foot wound that is red, swollen, smelly or turning black

Frequently asked questions

Can type 2 diabetes turn into type 1?

No, they are different conditions. But some people with type 2 eventually need insulin as their own production falls.

Can type 2 diabetes be reversed?

Some people bring their sugar into the normal range (remission) with significant weight loss and lifestyle change. It needs ongoing monitoring because it can return.

Is type 1 caused by eating too much sugar?

No. Type 1 is autoimmune and is not caused by diet or lifestyle.

Which tests diagnose diabetes?

Fasting blood sugar, HbA1c or an oral glucose tolerance test, interpreted by a doctor. Repeat testing is often used to confirm.
