Chest tightness: causes, warning signs & what to do
Written & medically reviewed by Dr. Sanjay Anbu, MBBS · Last reviewed
Medically reviewedA tight, squeezing or heavy feeling in the chest can come from the lungs, the stomach, muscles, anxiety — or the heart. Because heart causes can be serious, new chest tightness should never be ignored.
Possible causes
Common causes of chest tightness
- Heart — reduced blood flow (angina) or a heart attack, often with sweating or breathlessness
- Asthma — tightness with wheeze and cough, often at night or with dust
- Acid reflux — burning or tightness behind the breastbone after meals
- Anxiety or panic attacks — tightness with fast breathing and fear
- Strained chest-wall muscles after coughing, lifting or exercise
- Chest infections such as bronchitis or pneumonia
Self-care
What you can do now
- If there is any chance it is your heart, do not wait to see if it settles — seek help first
- Stop what you are doing and sit upright and rest
- If you have an inhaler prescribed for asthma, use it as your doctor advised
- Note when it happens (meals, exertion, stress) — this helps your doctor
Red flags
When to see a doctor urgently
- Chest tightness or pressure lasting more than a few minutes, especially with sweating, nausea, or pain in the arm, jaw or back — call your local emergency number now (108/112 in India)
- Sudden breathlessness, coughing blood or fainting
- Tightness that comes on with exertion and eases with rest (possible angina)
- Severe asthma that is not improving with your usual inhaler

Frequently asked questions
How do I know if chest tightness is my heart?
You cannot be sure at home. Tightness with exertion, sweating, nausea, breathlessness or pain spreading to the arm or jaw is more worrying. An ECG and blood tests are needed to check.
Can gas or acidity cause chest tightness?
Yes, acid reflux commonly causes burning or tightness behind the breastbone. But heart pain can feel similar, so new or severe symptoms need medical review.
Can anxiety cause a tight chest?
Yes. Panic and anxiety can cause chest tightness, fast breathing and tingling. It is still important to rule out heart and lung causes first.
